Triangle Tube has received two reports of death—one in 2015, and one in 2016 following a repair—due to carbon monoxide poisoning and 25 reports of carbon monoxide leaks associated with the recalled boilers.
Recalled boilers were manufactured between December 2011 and April 2019 and have serial number prefixes beginning with AE, PT, PA, PTE and PEA. CLICK HERE to identify if your product is recalled and schedule a free inspection and repair.

IF YOU ARE AN INSTALLER If you intend to service a recalled boiler, you must obtain approval from Triangle Tube prior to performing the inspection and repair. We ask that you please:
1. Visit the online portal that we have developed specifically for contractors: https://r2rportal.triangletube.com.
2. Register on the portal and if your customers have registered already and selected you as preferred installer, you will see their recall job. You are also able to register your customer for the recall through the same portal. If you’d rather provide this information via phone, please call us at 877-574-5036 from 8 a.m. to 5 p.m. ET Monday through Friday.
3. Triangle Tube will then confirm that you are eligible to conduct repairs.
